Self Management Activity Programs (SMAP)

This signature treatment program has been specifically designed by our very own Accredited Exercise Physiologists, for patients who suffer from chronic injuries and have demonstrated a reliance upon passive treatment such as massage therapy, physiotherapy etc.

Our evidence has  shown that by facilitating this program in the patient’s home, it produces a real life experience that they can apply in their lives over a longer term. It further highlights the positive impact that the environment that they perform prescribed exercises in, has on adherence and the durability of achieved outcomes.

The majority of our patients have indicated to us that they are less likely to attend a fitness facility and pay the associated membership fees themselves when they can have the luxury of performing prescribed exercises in their own home as part of the self management of their injuries.

  • Medical Case Conferencing with the Nominated Treating Doctor to achieve a “Final” or upgrade in medical certification upon completion

A Self Management Activity Program Is Needed When….

  • The injury is now chronic and disabling (usually >12 weeks post injury)
  • The injured worker has demonstrated that they are reliant upon passive treatment
  • Acute treatments are no longer resulting in significant functional improvement
  • Activity levels are not progressing and / or fluctuate widely
  • There is evidence of psychosocial risk factors and / or delayed recovery
  • The injured worker continues to report severe levels of pain 
  • The injured worker experiences regular injury flare ups or aggravations
  • The injured worker continues to seek reimbursement for ongoing gym memberships and / or associated travel costs.
  • Medical certificates are not reflecting the injured worker’s functional capacity or there is a lack of progress towards achieving the return to work goal.
